Ambulatory Surgical Centers (ASCs)
Licensing, oversight, and monitoring of Alaska ASC facilities
Overview
An Ambulatory Surgical Center (ASC) is a distinct entity that operates only for the purpose of providing outpatient surgical services to patients:
- Who do not require hospitalization
- For whom the expected duration of services would not exceed 24 hours following an admission
Who qualifies?
- ASCs are required to be licensed in the State of Alaska.
- ASCs may also be eligible for federal certification. To participate in Medicare, an ASC must meet federal requirements (see below) and be separately certified and approved.
How to sign up
Initial licensing
If you are interested in obtaining an initial license, please complete the following steps:
- Complete the ASC Licensing Application packet
- Pay the associated provisional licensing fee
- Provide an electronic copy of all policies and procedures
Submit all requested information to Health Facility Licensing and Certification for staff review. We will work closely with you to ensure your application meets all the licensing requirements, followed by an onsite inspection.

Renew your license
To renew your license, please complete the ASC Licensing Application and pay the renewal fee. You must submit the required information to Health Facility Licensing and Certification before April 1 in even-numbered years.
Biennial renewal fee: $1,300
Renewal applicationModify your application
If an ASC needs to modify previously approved applications, a modification application and associated fee must be submitted to Health Facility Licensing and Certification.
Relocation fee: $250
Miscellaneous modification fee: $250
Report leadership change
If the facility has a change in administrative leadership, a Change of Leadership form needs to be completed and submitted to Health Facility Licensing and Certification.

To participate in Medicare, an ASC must meet federal requirements and be separately certified and approved. Federal requirements for ASCs are outlined in the Code of Federal Regulations (CFR).
