Applications for Public Assistance

Exciting News!  Introducing Alaska Connect, our new client portal:

We are thrilled to announce the launch of our brand-new client portal Alaska Connect, designed to enhance your experience with the Division of Public Assistance. When you sign up through myAlaska or if you already have an existing myAlaska account, you can access your own personal space where you can apply for public assistance programs, renew your benefits, report changes and upload documents all in one place. 

We are excited to offer this new user-friendly option for Alaskans who use Public Assistance. More improvements are on the way as we modernize online services and improve the client portal, to include things such as reviewing notices, application status, benefits, and much more. Those Alaskans that do not wish to sign up for the portal can still access our services through our website, offices, and virtual contact center.

Medicaid Applicants, please note:

Apply for Medicaid BenefitsIncome-based Medicaid applicants are encouraged to apply at www.healthcare.gov. This is the fastest way to see if you’re covered.
Or visit the ARIES Self-Service Portal on my.alaska.gov.

An electronic application is not available for programs other than SNAP and Medicaid at this time. Use the links below to apply for other services:

  • Complete a paper Application for Services and send by mail, fax, email, in-person/lobby drop box, direct secure messaging (requires a DSM account), or through a fee agent. See our Contact Us page for more information.
  • Contact the Virtual Call Center 800-478-7778 (TDD/Alaska Relay: 7-1-1) and apply for benefits over the phone.

Copies of applications are available at local offices, and we can mail one to you if asked.

Older age or disability Medicaid applicants should only use the following (paper) application at this time:

Other Applications

Review or Recertification

  • The Eligibility Review Form (GEN-72) can be used for most programs. The division sends a prefilled review application form with the standard recertification or review notice.
  • Please note the reporting requirements for our programs, which can be reported directly to your local office, with a GEN-72 or other form.
