This secure application allows you to directly review immunization history and share official vaccination reports for health, school, travel, and more.
- Download the free app to your mobile device from the App Store , Google Play Store, or visit Docket's web version.
- Follow the verification steps
- Export and download PDFs of records directly to keep or share as you choose
Note: You must enter demographic information (name, date of birth, and legal sex)
