Download your record from the Docket app or its
web version. This secure application allows you to directly review immunization history and share official vaccination reports for health, school, travel, etc.

Search for your immunization records:

You must enter demographic information (name, date of birth, legal sex).

Access your records:

Export and download PDFs of records directly to keep for records or share as you choose.

Request records through your health care provider. Patients can request copies of their immunization records through their health care provider. All immunization service providers in Alaska are required to utilize VacTrAK to document vaccine administration. VacTrAK does NOT provide access to patients, only to health care providers.

Request records by fax or mail. The Alaska Department of Health can fax or mail vaccination records or the WHO International Certification of Vaccination (Yellow Cards). Requests are completed via the
State of Alaska Immunization Request Form and submitted by fax or mail.
- Requests must include a legible copy of a government issued photo ID that identifies the requester.
- Mailed vaccination records are sent by USPS 1st class and take a minimum of 5-7 business days to process.
- Individuals requesting a Yellow Card must include a note on the request stating that it is required for international travel.
- Records for minors cannot be released directly to parent(s)/guardian(s). Their records will only be released to a licensed daycare, school, or health care provider.
