Nursing Unit

The Nursing Unit is responsible for the oversight and management of the following processes.

Children with Complex Medical Conditions (CCMC) waiver program assessments and Support Plan review.

• Interested in applying for a waiver program, Community First Choice (CFC) or Personal Care Services (PCS)?
• If you are at home or in a facility, you will need to start with contacting the Aging and Disability Resource Center for a person-centered screening and options counseling. You will select a Care Coordinator to assist with your application.
• You will also need to select a PCS agency for Personal Care Services.

Facility assessments. ALI and APDD waiver programs, CFC, PCS assessments for individuals in Nursing Homes, Hospitals, Psychiatric or Correctional facilities

• For facility assessments, we will work the discharge planner to collect medical records and schedule your assessment. Currently, we continue to practice all recommended COVID 19 precautions, all assessments are conducted via using the platform Zoom.
