Supply Request  Fill the form and fax to either laboratory to fill your needs for collection supplies. We also offer validation panels to users of the Abbott ID NOW SARS-CoV-2 assay to help in training and to provide confidence to your testing staff.

SARS-CoV-2 Test Request Form  Please use the request form for all SARS-CoV-2 requests.  Fill out the form completely and accurately.  The specimen should also have two identifiers that match exactly (ex. full patient name and date of birth) to the information on the form.  Any submission errors may delay testing.
