Project HOPE evaluation forms

If you can not submit your form digitally, you may download and submit a hard copy using the links below.

Please submit forms to ProjectHOPE@alaska.gov and to Becky Porter (UAA evaluation team) at rrporter2@alaska.edu. Or, mail them to: Alaska Office of Substance Misuse and Addiction Prevention at 3601 C Street, Suite 760

Online printable forms

• If you used Narcan® in response to an overdose, submit the Report Back Public Evaluation.
• Getting ready to be trained and receive a Narcan® kit? If you are not a Project HOPE trainer or first responder, submit the Pre-Post Survey for General Public.
• If you are being designated as a trainer within an agency or organization that offers Project HOPE, please submit the Pre-Post Survey for Trainers before your training. You also need to remember to submit your Distribution Tracker Form
• If you are a first responder, submit the Pre-Post Survey for First Responders before and after you receive training. You’ll also submit distribution tracker forms, found above.
