Diabetes Self-Management Education and Support (DSMES)

People with diabetes are at a higher risk for severe health complications, such as blindness, kidney failure, heart disease, stroke, and amputations. Fortunately, better health management can help people with diabetes live longer and healthier lives.

Diabetes Self-Management Education and Support (DSMES) services teach participants skills to take ownership of their daily diabetes management, including how to check blood sugar, develop routines to take medication, incorporate healthy eating and activity into daily life, and cope with stress and diabetes distress. Education is provided by a healthcare team trained to help people with diabetes set, reach, and maintain their health goals.

Benefits of DSMES participation can include:

  • Improved hemoglobin A1C levels (the 2-3 month average of blood sugar levels)
  • Improved control of blood pressure and cholesterol levels
  • Higher rates of medication adherence
  • Fewer or less severe diabetes-related complications
  • Healthier lifestyle behaviors
  • Decreased health care costs.
