Data Collection Forms

The MCDR program uses standardized forms to collect data elements for all maternal and child deaths. The Child Death Review form is used to enter data into the National Fatality Review Case Reporting System (NFR-CRS). The Maternal Death Review form is used to collect data and is currently entered into an in-house database. This data will be migrated to the Maternal Mortality Review Information Application (MMRIA) System as soon as access is approved.
