Fees-for-Service

The Alaska State Public Health Laboratories offer some services that include fees. Health care providers may use these services once a memorandum of agreement (MOA) is on file at the Alaska State Public Health Laboratories. Download and complete the necessary forms below for the testing desired and email to theresa.savidge@alaska.gov.

*Note: There is no charge for filing the MOA. However, it is recommended that MOA's are on file prior to submitting specimens to reduce processing delays.
