Assessment and level of care determinations
Who we are
The SDS Assessment Unit is responsible for the assessment and level of care determinations for these waiver types:
- Adults Living Independently (ALI)
- Adults with Physical and Developmental Disabilities (APDD)
- Community First Choice (CFC)
- Personal Care Services (PCS)
What we do
These waiver programs allow people who meet a nursing facility level of care (NFLOC) to choose between home and community-based services or institutional care. To qualify, people must meet Medicaid income and resource limits through the Division of Public Assistance (DPA) and pass a level-of-care assessment by SDS. Trained professionals talk to people in person or on video chat, and findings are reviewed by a nurse supervisor.
Waivers include:
- ALI: For adults 21+ needing nursing facility care
- APDD: For adults 21+ with developmental disabilities and nursing needs
- CCMC: For children under 22 with complex medical needs requiring hospital-level care
A support plan of services is made by the person’s care coordinator.
