Overview 

The Community First Choice (CFC) program, also known as 1915(k), provides long-term services and supports to eligible Alaskans. This program emphasizes participant autonomy in managing their care while enabling the State to receive an increased share of federal funding for these services.

CFC can include Personal Care Services (PCS), Personal Emergency Response Systems (PERS), case management, and chore services.

Who qualifies?

To join the CFC Program, you must meet federal rules for financial and medical needs, including needing care usually provided in an institution.

The first step is to contact your local ADRC or DDRC. It can help to have a family member or friend help you with the call or visit.
