Overview

Through the Diabetes Self-Management Education and Support (DSMES) program, people learn how to manage their diabetes with skills like checking blood sugar, taking medicine on time, eating healthy, staying active, and coping with stress and other diabetes challenges. The DSMES team is trained to help people set, achieve, and maintain their health goals.

  • Improved hemoglobin A1C levels (the 2-3 month average of blood sugar levels)
  • Improved control of blood pressure and cholesterol levels
  • Improved ways to take your medicine at the right doses and times
