HIV/AIDS Linkage to Care
Connects patients to intensive support including finding an HIV care provider
Overview
Linkage to Care (L2C) staff connect people living with HIV to medical care, focusing on newly diagnosed patients or those not in care. Support is made to help each person with their individual needs and may include:
- Help finding medical providers
- Assistance accessing HIV care and treatment
- Connections to social services including risk-reduction counseling
- Links to long-term medical care management
In-person support is available in Anchorage, while patients outside Anchorage receive support on phone or video calls.
Who qualifies?
Any person residing in Alaska who meets one of the following criteria is eligible for Linkage to Care (L2C) services:
- Diagnosed with HIV recently
- Pregnant and living with HIV
- Out of HIV care, as identified by a healthcare provider (for example, no CD4 or viral load test, or no HIV care appointment in the past 12 months)
High-risk HIV-negative individuals, including those:
- Engaging in repeated sexual encounters with someone known to have HIV
- Injecting drugs or sharing needles with someone known to have HIV
- Participating in transactional sex work
- Engaging in sexual contact with anonymous partners from websites or apps
If someone with HIV does not meet these criteria, they will be referred to long-term HIV/AIDS case management services through local organizations.
