Long-Term Care Authorization
Required approval to get into a nursing home or other long-term care facility
Overview
We review, approve or deny requests for nursing home placement and maintain records related to these actions. We also provide oversight of the state’s Pre-admission Screening and Resident Review (PASRR) process.
Who qualifies?
Everyone must be screened for mental illness, intellectual disability, or related conditions before entering long-term care to make sure the person's needs are met.
Prior authorization is needed from your medical team to qualify for these services.
How to sign up
Long term care authorization request forms may be submitted to SDS through the Direct Secure Messaging (DSM).
If you do not have a DSM account you can create one through their website or by emailing support@ak-ehealth.org.
