This secure application allows you to directly review immunization history and share official vaccination reports for health, school, travel, and more.

• Download the free app to your mobile device from the App Store  , Google Play Store, or visit Docket's web version.
• Follow the verification steps
• Export and download PDFs of records directly to keep or share as you choose

Note: You must enter demographic information (name, date of birth, and legal sex).

If you are unable or prefer not to use Docket to access your immunization records, you may contact your health care provider or submit the Immunization Record Request Form. Please note that the Immunization Record Request Form is the only form accepted by the Alaska Immunization Program for processing immunization record requests.
