Licensing, oversight, and monitoring of Alaska HHA facilities
Overview
Home Health Agencies (HHAs) provide services that cover a broad range of care provided in the home environment.
HHA services may include part-time skilled nursing care, physical therapy, occupational therapy, speech-language therapy, home health aide services, and medical social services. HHA facilities may also provide durable medical equipment (such as wheelchairs, hospital beds, oxygen, and walkers) and medical supplies.
Who qualifies?
- HHAs are required to be licensed in the state of Alaska.
- HHAs that meet federal requirements (see Resources, below) may also be eligible for federal certification.
How to sign up
Initial licensing
If you are interested in obtaining an initial license, please complete the following steps:
- Complete HHA Licensing Application packet
- Pay the associated provisional licensing fee, based on the number of full-time employees (FTEs) and number of branches/subunits
- Provide an electronic copy of all policies and procedures
Submit all requested information to Health Facility Licensing and Certification. Your application will be reviewed by our unit staff. We will work closely with you to ensure your application meets all the licensing requirements, followed by an onsite inspection.
Provisional licensing fee (based on FTEs, as follows):
- $1,000 (for 5 or fewer FTEs)
- $1,500 (for 6-15 FTEs)
- $2,000 (for 16-50 FTEs)
- $3,000 (for 51+ FTEs)
Plus $500 per branch/subunit

Renew your application
To renew your license please complete the Hospice Licensing Application and pay the renewal fee. You must submit the required information to Health Facility Licensing and Certification before April 1 of even-numbered years.
Biennial renewal fee (based on FTEs, as follows):
- $2,000 (for 5 or fewer FTEs)
- $3,000 (for 6-15 FTEs)
- $4,000 (for 16-50 FTEs)
- $6,000 (for 51+ FTEs)
Plus $1,000 per branch/subunit
Renewal applicationModify your application
If the facility needs to modify previously approved applications, a modification application and associated fee must be submitted to Health Facility Licensing and Certification.
Relocation fee: $150
Miscellaneous modification fee: $250
Additional branch/subunit fee: $500 per additional location
Plus any fees associated with additional FTEs:
- $1,000 (for 5 or fewer FTEs)
- $1,500 (for 6-15 FTEs)
- $2,000 (for 16-50 FTEs)
- $3,000 (for 51+ FTEs)
Report leadership change
If the facility has a change in administrative leadership, a Change of Leadership form needs to be completed and submitted to Health Facility Licensing and Certification.

Federal requirements
In order to participate in Medicare, an HHA must meet federal requirements and be separately certified and approved.
