Submit a Lab Test
Instructions for submitting specimens to the Alaska Public Health Laboratories
Prepare
Select forms
You must submit a completed test request form with each patient (or animal) sample. Select the lab request form(s) you’ll need from the Forms page.
Gather supplies
Use the Supply Request Form to request collection kits, ambient temperature shipping boxes, and other testing supplies.
Please note:
- Exclusions may apply depending on the test. For more information, please contact us.
- Alaska State Public Health Laboratories do NOT supply blood collection tubes or biohazard bags.
- Specimens collected in expired collection devices or transport materials will be rejected as “unsatisfactory” and will not be tested. Please monitor expiration dates carefully.
Collect
Find and follow the collection instructions for the test(s) you plan to submit.
Label
Each patient submission must include:
- properly labeled specimen(s) and
- a properly completed request form
Label specimen(s)
- Adhere a label directly on each specimen with the same exact unique identifiers used on the Lab Request Form.
- A minimum of 2 unique identifiers are required:
- Patient’s Full Name or Alternate ID (ex: chart number, medical record number, prison ID); and
- Patient’s Date of Birth
- A minimum of 2 unique identifiers are required:
Complete a lab request form
- Complete all required fields* on the appropriate lab request form
- Use the same exact unique identifiers used on the specimen label
Ship
Package and ship all specimens according to federal, state, and local shipping regulations. This is required to be done by someone with prior training in shipping dangerous goods. If you have questions about proper shipping practices, please contact us.

Shipping address
The correct shipping address will be on each test instruction page and the lab request form you submit with your test.
Note that third party shipping vendors will not accept a PO box address.
