Overview

The Alaska Medicaid Coordinated Care Initiative (AMCCI) is a voluntary program that provides one-on-one case management services. AMCCI services include coordinating care, scheduling appointments, addressing barriers, and making referrals.  

This program helps patients find a doctor's office or clinic to better manage their health needs wherever they live. People who are referred to this program become “AMCCI members.”

Who qualifies?

Alaskans who receive medical assistance, such as Medicaid, Denali KidCare, or chronic and acute medical assistance, may be eligible.

How to sign up

If you know someone who may benefit from this program or if you are interested, please complete the Member Referral form linked to this website. 

Providers may refer patients using the provider form. Any person receiving Medicaid services who is interested may also fill out a form to refer themselves.
