If you would like to request a copy of a survey that has not been uploaded to this site or for any surveys prior to January 1, 2018 please send a request to dhcs.hflc@alaska.gov. Please include the name of the facility as well as a date or date range. Request will be processed within 10 business days.

Facility inspection reports and complaints older than 25 months are accessible via the Alaska State Libraries Archives & Museums digital collections portal.
