Applying for a program other than SNAP?

Apply for Medicaid online through healthcare.gov or my.alaska.gov
For other programs, such as the Temporary Assistance Program, Adult Public Assistance, General Relief Assistance, and Chronic & Acute Medical, complete this paper Application for Services (GEN 50C) and return to the Division of Public Assistance by:

Emailing it to: hss.dpa.offices@alaska.gov; or
Finding the nearest drop off location.

Be prepared: You may need to provide the following information about the people in your home:

Name
Date of Birth
Citizenship or Alien Status
Social Security Number
Income
Assets and Resources
Housing Costs
Utilities
Childcare Costs
Other Household Circumstances

Applicants have the right to file an application to secure their benefit start date with just their name, address, and signature. However, if you submit your application to us without filling out the questions that apply to your case, you will have to answer them before you can get benefits.
